• Lions Club Activity Calendar Listings 2021

    Welcome to our ONLINE FORM to sign up for the Annual Lions Club Calendar with Listings which will be produced for your activity: Birthdays, or Anniversaries, or "In Memory of" dedications with dates from June 2021 through May 2022. (1 Year)

    COST: $6.00 Per Calendar-includes 1 FREE listing
    COST: .35 Cents per Additional Listing
    DEADLINE: Due by March 15, 2021
    AMOUNT DUE: You will be contacted by the Norton Lions Club for payment. For example, you buy a calendar with 1 listing. Cost: $6.00. Example: You buy a calendar with 6 listings. Cost: $7.75. (Calendar $6, 1 listing free, 5@.35=1.75)

    1. Fill out your name and address.
    2. Fill out your phone number and email address so we can contact you. DO NOT put dashes in the phone number. Example: 7858710095
    3. Fill out how many listings you would like in all i.e. 4 or 5 or 9 (be sure to include ALL listings)
    4. Each type of listing has its own section to fill out: i.e...List all BIRTHDAYS under that section, list all ANNIVERSARIES under that section, list all 'IN MEMORY OF' under that section. We DO NOT need any year, just the month and day.

    EXAMPLES: You have 2 Birthdays you want to list with information listed in this order: Name - Month & Day of the Birthday, (NEXT LINE) Name - Month & Day of the Birthday like this:
